Appointments
Appointments may be scheduled by calling the office where the physician is located. Please bring all insurance information along with a current list of medications on your first visit. Arrive 15 minutes prior to your scheduled appointment time to complete a patient information form and health questionnaire.
Insurance
First Care files all insurance claims. First Care is a participating provider for SHARP, Blue Cross-Blue Shield, First Source, USAble and most other health insurances. First Care accepts assignment for Medicare and Medicaid. Please remember to bring your insurance card each visit to expedite the paperwork process and ensure that our records are correct. Your insurance cards will be copied and returned to you.
Payment of deductibles and co-pays is expected at the time of service. Patients are responsible for any balance not paid by insurance.
If you do not have insurance, payment is expected at the time of service. Cash, check, Discover, MasterCard, or Visa is accepted. Patients unable to pay at the time of service are asked to make a deposit and sign a payment agreement for monthly payments. We ask that the balance be paid within 60 days.
